How to add medical records in minutes (and why it matters).

Medical records detail your pet's medical history and give notes on treatment they’ve received. This allows us to process your claims quickly. Without your pet’s medical records, we might not be able to process your pet’s claims at all.

To add medical records:

1. Go into the Account section of your app or your online account.

2. Click the “Add medical records” button, and follow the steps.

IF THIS IS YOUR FIRST YEAR AS A FETCH MEMBER:

What medical records do you need?
Please ensure detailed medical records are uploaded to your account for the 12 months prior to the start of your pet’s Fetch policy.

I don’t have all of my pet’s medical records. What should I do?
Contact your vet to request them as soon as possible. To make sure you get the right kind of medical records, tell them you need “SOAP notes.” The records you receive should include any exam notes and lab results.

What if my pet hasn’t been to the vet in the last 12 months?
Contact your vet to schedule a checkup as soon as possible. Then, upload detailed medical records from that visit.

What if I’ve had my pet for less than 12 months?
No problem — just upload detailed medical records from every vet visit since you’ve had them.

IF YOU’VE BEEN A FETCH MEMBER FOR MORE THAN A YEAR:

What medical records do you need?
Please submit detailed medical records from all of your vet visits during the last two years. Your vet may refer to these as SOAP notes.
